The Effects of Age and Circadian Rhythm on Theophylline Pharmacokinetics in Normal Subjects

Theophylline metabolism is influenced by a wide variety of biochemical, pathological and environmental factors. These include diet, smoking, other drugs and various disease states. We investigated the effect of advanced age on theophylline disposition. Theophylline clearance rate was higher (43.97 vs 32.97 ml kg−1 hr−1, p<0.001) and the elimination half-life was shorter (6.98 vs 8.14 hr, p<0.02) in the geriatric versus young age group.

We also studied the influence of circadian rhythm on theophylline disposition. In normal male volunteers, age 20-25 yr, there is a significant decrease in the serum theophylline concentration and the area under the serum theophylline concentration-time curve (AUC) during the night. Pharmacokinetic rate constants cannot be considered a priori to be identical for theophylline throughout a 24-hour period.
