Structural failure investigations in Australia
Thirty structural failures in Australia are discussed and theories are proposed about their possible causes. Many lessons are to be learned from a study of these causes. Clearly, direct design errors are the principal underlying cause, but inadequate methods of construction and/or operation also are strong contributing factors. The role of the expert is discussed and the importance of an impartial and scientific approach is stressed.
