Abstract:

Intermediate care (IC) refers to those services that bridge the gap between hospital and home. The care is often fragmented and provided by different agencies with many different and varied ports of entry. In many health economies, IC is seen as a discrete entity, jealously guarded by the relevant primary care trust. The model of care pioneered within the area of Shepway is as a result of the forward thinking from the acute trust, the primary care trust and social services.
