Redesigning a local authority residential care home to provide an intermediate care resource

Abstract:
Key national policy drivers have identified the need for change in the way that services are delivered, with shifts to interagency redesign and the need for a whole system approach to early intervention, prevention, rehabilitation and enabling. A redesign of a wing of a Scottish Borders local authority residential care home to provide an intermediate care resource was carried out in 2008–09 and fully evaluated. This paper presents the main findings of the evaluation and concludes that the redesign has demonstrated an effective model that can be a transferable framework to deliver intermediate care in other residential care homes.Keywords: INTERMEDIATE CARE; SERVICE REDESIGN; RESIDENTIAL CARE HOMES; SCOTTISH SYSTEM; NATIONAL POLICY DRIVERS
