Abstract:

How effective is your organization's efforts to reduce falls? One way to find out is by conducting your own fall reduction tracer. This column includes a scenario, sample tracer questions, and tips about how to conduct a fall reduction tracer in a home care environment. Although this article's example discusses home care, other health care settings can benefit from conducting their own fall reduction tracer as a performance improvement strategy.
