Abstract:
Suicide is a significant patient safety concern for health care organizations; therefore, the assessment and identification of suicide risk are crucial to protecting these at-risk individuals. The Joint Commission developed the suicide prevention program specific tracer for the hospital and behavioral health care programs, but all health care organizations can benefit from conducting their own tracer as a safety strategy. This particular tracer evaluates the effectiveness of an organization's suicide prevention strategy and identifies processes and possibly system-level issues contributing to suicide attempts. This article includes a tracer scenario with sample questions and a tips checklist on how to conduct this tracer on your own.Document Type: Research article
