Abstract:

Health care organizations are required to have a process or system in place to provide and support safety practices as well as report and analyze problems and proactively take action to correct failures. The Joint Commission's Standard LD.04.04.05 emphasizes the need to integrate patient safety activities, both existing and newly created, with the organization's leaders, who are ultimately responsible for this integration. This article discusses how Joint Commission surveyors will check compliance for this standard and offers advice on what organizations can do.
