It is imperative that physicians confirm the correct name, strength, dosage, and route of every medication before it is administered on a patient. Medication containers must be accurately labeled, and the transfer of containers from one location to another must be carefully executed to prevent any contamination of the medication. The Joint Commission's National Patient Safety Goal 03.04.01 requires that all medications, medication containers (for example, syringes, medicine cups, basins), and other solutions on and off the sterile field be properly labeled.
