Abstract:

Medication errors are among the most common type of medical errors, harming at least 1.5 million people each year, 400,000 of them in hospitals. Many healthcare organizations are using surveillance strategies to track and analyze medication errors, so that process changes can be made to prevent future errors. Methods such as voluntary reporting, manual chart reviews, automated systems, and direct observation are discussed.
