Abstract:

Skin tears can be caused by improper patient transfers (improper lifting, turning, or positioning techniques), falls, intravenous catheters, and improper (harsh or quick) tape removal from the skin. These relatively common injuries are preventable. Five strategies for reducing the risk of skin tears include the following: assess patients risks for skin tears, educate the patient, family, and staff on preventive practices, use proper transferring techniques, use proper techniques to remove tape from patient's skin or use alternatives to tape when possible and protect and support the patient's extremities.
