Bipolar disorder in later life

Abstract:
There has been growing awareness regarding the manifestations of bipolar disorder among older adults, owing to both changes in national demographics and developing sophistication in the treatment of bipolar illness. As the effects and impact of bipolar disorder are multidimensional, suffering and burden related to illness can be expected to affect not only the older adult with bipolar disorder, but also their families and society at large. Clinicians struggling to meet the needs of geriatric patients with bipolar disorder have few evidence-based studies on which to base treatment decisions, and must often contend with issues of psychiatric and medical comorbidity in these vulnerable individuals. This article will address geriatric bipolar disorder epidemiology, health-service utilization, clinical presentations of bipolar disorder among geriatric patients, the entity of secondary mania or mania related to identified medical/neurological pathologies, and treatment and management issues of older adults with bipolar disorder. Finally, ongoing challenges and unmet needs with respect to geriatric bipolar disorder will be discussed.Keywords: antipsychotic; bipolar disorder; elderly; epidemiology; mania; medications; mood stabilizers
