Abstract:
Recent advances in the understanding of the pathophysiology and aggressive treatment of rheumatoid arthritis, ankylosing spondylitis and psoriatic arthritis have dramatically changed the outcomes of patients with these diseases over the past 7 years. Much of the progress in therapy is due to the change in the treatment paradigm in which early, aggressive therapy has become the norm and the introduction of therapies that target the disease processes, specifically the overproduction of tumor necrosis factor-α. Etanercept was the first anti-tumor necrosis factor-α therapy approved for use in rheumatoid arthritis, juvenile rheumatoid arthritis, ankylosing spondylitis and psoriatic arthritis. It has been shown to have an excellent risk:benefit ratio in clinical trials as well as postmarketing surveillance. However, there has been limited information regarding the safety and efficacy of etanercept in patients aged 65 years or over, which is the focus of this review. Keywords: ankylosing spondylitis; anti-tumor necrosis factor; etanercept; juvenile arthritis; psoriatic arthritis; rheumatoid arthritis
