Summary

Hypertensive patients with additional risk factors such as diabetes, hypercholesterolemia, left ventricular hypertrophy, and obesity require much more aggressive goals and powerful drug therapies than uncomplicated hypertensives. In addition, elderly hypertensive patients at high risk are most often predisposed to left ventricular dysfunction. To date no guidelines for the management of these patients are available in Asia. This article reviews the findings of the Chin-Shan Community Cardiovascular Cohort study, a longitudinal cohort study from Taiwan, which revealed that the incidence of cardiovascular events increases as the patient's hypertension becomes severe. The article suggests routine monitoring of microalbuminuria and a complete lipid profile on patients should be done in order to identify possible risk factors and the course of treatment.
